| Abstract |
| The posterior approaches to the sciatic nerve block require to move the patient in the lateral position. This is unpractical and may be painful in the patients with leg fracture. The anterior approach overcomes this problem allowing the block performance in the supine position, without need to move the patient or the leg. However we have demonstrated that anterior sciatic approach carries the risk of vessel damage, since the needle necessarily crosses a richly vascularized area close to the femur, so this block may be not the best choice in a already seriously injuried leg. We have developed a novel sciatic nerve approach, the postero-lateral gluteal approach, which allows a very proximal nerve block with the patient remaining in the supine position. The linear probe is positioned transverse in the posterolateral gluteal area, proximally and medially to the greater trochanter (fig. 1, 2, 3). The corrispondent US immaging is showed in fig 4 and 5. The posterolateral gluteal sciatic nerve block proved to be a viable option to perform a proximal sciatic anesthesia in the supine position in the trauma patient. Videoclips 1 and 2 show the ability to reach the nerve with the needle, and the perineural local anesthetic spread control.
