Abstract
Paravertebral sonography of lumbar spine was previously confined from back side in order to identify bony landmarks, like spinous processus, transverse processus or articular processus. Because of narrow and complex intervertebral space, sonographic window to central components like epidural space, cauda equina or vascular structures are, at its best, very limited. On the contrary, intervertebral disk provides good sonographic window, thus enable us to identify spinal canal and nerve root region of lower lumbar spine by transdiscal insonation of ultrasound beamiAnterior Transdiscal Sonography: ATDSj. In this preliminary imaging study on the adult patient with lumbar disk hernia (L5/S1), preoperative ATDS revealed hypoechoic image of disk hernia displaced cauda equina and nerve root, and surgical removal of the hernia recovered the symmetrical appearance of cauda equina and resulted the disappearance of painful symptom in left lower extremity.
Clinical Significance
ATDS of lumbar spine may reveal pathological disorders in spinal canal; i.e. lumbar disk hernia, hematoma or tumor. Transdiscal insonation of ultrasound beam may also be used to guide the real time advancement of the needle for transdiscal interventions for pain medicine; like celiac plexus block, inferior mesenteric plexus block, superior hypogastric plexus block and lumbar sympathetic plexus block etc. Although further studies may need to evaluate usefulness, safety and limitation of this imaging technique, this novel sonographic window will provide an expanded view for pain clinician.
